WHO IS ELIGIBLE?
Children must be 5 on or before March 1, 2013.
Community Membership is required in order for your child to participate.
(Memberships available on registration night.) |

Completed registration form (forms will also be available at the Open House)
Chequebook to pay the $45 non-refundable registration fee, $20 non-refundable community membership fee and cleaning deposit (cheque to be returned upon completion of cleaning commitment).
Original birth certificate of the child
Alberta Health Care number of the child |
